Our vision for Adult Social Care
We want people in Sefton to live happy, healthy and independent lives for as long as possible. When support is needed, we want people to be able to access high-quality care and services that make a positive difference.
Our aim is to help people stay connected to their communities, maintain their independence and have choice and control over their care and support.
Our Ambition
We are committed to providing care and support that:
- Promotes independence, choice and control
- Provides high-quality support for people with a wide range of needs
- Offers the right support at the right time through prevention and early intervention
- Delivers fair and equitable outcomes for all
- Supports unpaid carers to achieve their own goals and wellbeing
- Helps keep people safe from abuse, neglect and discrimination
- Brings health and social care services together to provide joined-up support
- Values and supports our workforce
- Makes it easier for people to access services and tell their story once
- Works with care providers to offer flexible, high-quality local services
Better at Home
Our Better at Home approach focuses on helping people live independently in their own homes and communities for as long as possible.
We know that many people want to remain at home, close to family, friends and local support networks. When needed, we can provide care and support, information and advice, equipment, technology, therapy services and community-based support to help make this possible.
Over the coming years, we will continue to develop services that help people stay independent, reduce unnecessary reliance on long-term residential care and provide more choice and flexibility.
This includes:
- Extra Care Housing
- Sheltered and supported housing options
- Reablement services that help people regain independence after illness or injury
- Home care services
- Early help, advice and information
- Support for unpaid carers
- Assistive technology and equipment
- Integrated health and social care services
Working Together
Achieving our vision means working closely with residents, carers, communities, partners and providers.
We will continue to invest in a skilled, valued and innovative workforce and develop services that respond to the changing needs of local people.
